As you state it is every page in the admin section then it can not be an incorrect user else you would not see the admin pages.
error reporting won't help as it is not an apparent error , more like insert by host or a module or admin template.
thereby discounted previous suggestions by others.

the word doc contains partial info and I couldn't trace the owner of the adsense code .
