The template of the login module is modules/mod_login/tmpl/default.php. However, do not modify that file because it is one of the Joomla core files and may change in the next update. The contents of the login module - and logout - can be modified by creating an override. The Beez3 template has an example of an override in templates/beez3/html/mod_login/default.php.
