Or of course i would certainly be able to help and provide such a plugin, which i have already developed for our own default contact form for adding a telephone field and soon to be more, this will be released as a downloadable plugin shortly.
In the meantime i'm more than happy to provide it for the use above. Also no database tables need to be touched since its merely xml fields that are added and these are saved as parameters, you then are left with a system that can be upgraded with no data loss.
